Patient Forms

Upon entering our office, our receptionist will greet you and welcome you as part of our family. You’ll be asked to complete our new patient forms, which can be found in our New Patient Center. This paperwork provides us with important information about your health history and current condition.

Consultation

Next, you’ll meet with one of our doctors to discuss your health concerns, symptoms, and goals. This consultation allows us to better understand your condition and determine whether chiropractic care is the right fit for your needs.

Examination

After the consultation, you’ll receive a thorough chiropractic examination. This may include testing your reflexes, range of motion, posture, and performing standard neurological, orthopedic, and physical assessments.
Nothing will be done without your consent.

Diagnostic Imaging

Depending on your condition, diagnostic imaging may be recommended to help us evaluate your spine and rule out any serious issues.
We only order X-rays or MRIs when necessary, with a preference for MRI when advanced imaging is needed to fully assess your condition.

Report of Findings

Once all information has been reviewed, your doctor will go over the results and answer the three most common questions:

  • Can you help me?

  • What do you recommend to get better?

  • How much will it cost?

We’ll discuss your history, exam results, imaging (if applicable), and your health goals to determine the best course of action. If your condition requires care from another provider, we’ll let you know and help with referrals. If we believe chiropractic care can help, we’ll recommend a personalized treatment and wellness plan.

Treatment

In most cases, patients receive their first treatment during the initial visit. This may include chiropractic adjustments, soft tissue therapy, or other physiotherapy techniques. Treatment is always interactive—you’re encouraged to share preferences and ask questions so we can tailor your care to your comfort level.

Wellness Program

Before you leave, we’ll provide you with guidance for at-home care. If you’re in pain, this may include instructions for ice or heat therapy, activity modifications, or specific stretches and exercises. If you’re interested in long-term wellness, our team will help you develop healthy habits to support your goals.

Because every person is unique, your wellness plan will be customized to fit your body, lifestyle, and objectives. A well-rounded wellness lifestyle may include:

  • Maintaining proper posture and spinal function

  • Eating a healthy diet and taking appropriate supplements

  • Managing weight

  • Reducing stress and improving sleep habits
